I know some steroid use can hamper healing, but I wouldn't think that would be an issue with him.
If I had to guess, I would say STD.

That sucks it's not a contact dermatitis. Even the kind that spreads fast if you start popping benadryl every six hours, it usually clears up in 2 days

Josh said at one point that he unwisely tried a minor "self-surgery" on it (maybe with tweezers or a pocket knife, I don't know) and that made it worse.

There's folliculitis, but I can't see how it can get to be that serious. Josh said early on he thought he had an ingrown hair on his leg or something similar. maybe it's something embarrassing so he doesn't want to talk about it.

I have read that mycoplasma gondii and some other mycoplasmas (these boogers are much smaller than a bacterium or virus) are causing a much greater rate of skin infections that in previous decades. Many reasons for this, too complicated to explain here.
Some of those infections can be chronic and quite debilitating. Knew a guy in his 30s who could no longer climb stairs it got so bad in his knee. There is a treatment though and it takes 1-2 weeks.

If he has MRSA, they are filling him full of powerful antibiotics.......I have had that a couple of times when I had a foot infection and it is no laughing matter.

Some interesting information from the CDC about MRSA. I had previously thought it only affected people who had just had surgery. Apparently, athletes are among those at higher risk:
"Alarmingly, MRSA is also showing up in healthy people who have not been hospitalized. This type of MRSA is called community-associated MRSA, or CA-MRSA.
CA-MRSA skin infections have been identified among certain populations that share close quarters or have more skin-to-skin contact. Examples are team athletes, military recruits, prison inmates, and children in daycare. But more and more CA-MRSA infections are being seen in the general community, especially in certain geographic regions.
CA-MRSA is also more likely to affect younger people. In a study of Minnesotans published in TheJournal of the American Medical Association, the average age of people with MRSA in a hospital or health care facility was 68. But the average age of a person with CA-MRSA was only 23."

I have a friend from Australia who sells colloidal silver as an antibacterial agent in Vietnam (the hospitals often use it to wipe down counters, beds, and tables) and he claims it has been used successfully as part of some MRSA treatments:

The weird thing was he came back, got that double to win one of their only series against a decent team (Cleveland), and then couldn’t play again.
To be honest, I think it’s what others suggested, a staph infection.
